$135 per 53-minute session
Insurance
Services may be covered in full or in part by your health insurance or employee assistance. Services are covered by the following insurance plans.

- Independent Health
- Univera
- Nova
Payment
Cash, check and all major credit cards accepted for payment.
It is your responsibility to know the extent of your insurance coverage. Should your insurance claim be denied for any reason, you are responsible for direct payment of fees. All co-payments and non- insurance covered self-pay fees are due in full at the time of services. There are no exceptions to this policy.
****All co-payments for services provided to a child are the responsibility of the person bringing the child to the visit, even if you have a separation or divorce agreement that states otherwise. It is up to you to work out financial responsibility with the other parent.
Insurance plans that require pre-authorization or referrals are your responsibility. If you not obtain authorization, services will be billed on a self-pay basis until proper authorization is obtained.
Cancellation Policy
If you do not attend your scheduled therapy appointment, and you have not notified me with at least 24 hours notice, you will be required to pay a $50 missed appointment fee. This is not intended to be punitive, but is necessary to off-set the cost of reserving a 53 minute session with me.
